Days Off Calculator User Evaluation

The Days Off Calculator is only as good as its ability to meet your needs.
Because it's freeware, we don't get much user feedback -- yet feedback is
our best way to know how to improve future versions.

We always welcome your comments, questions, and suggestions by phone
(504/244-9234), mail, or fax (see address and fax number below).  But you
can also help by taking a moment to fill out and send or fax us this form.

We do not keep a list of registered users, nor will you be notified
automatically of future versions.  However, future versions will be listed
in the McTrans and PC-Trans magazines.  (These are computer magazines
specifically for transportation planners -- BOTH FREE -- and we recommend
them highly!  To subscribe, call McTrans at 800/226-1013 and PC-Trans at
913/864-5655.  Both McTrans and PC-Trans will charge a small handling fee
if you order the calculator from them, but if you contact us directly,
we'll send it free.)
